Obtaining an ID Card


For Military Personnel:
  • Rank Change: Have Orders because it may not reflect in DEERS and two forms of identification.
  • New ETS or Enlistment: Contact your local DEERS office to verify the change is reflected on your record. Within milConnect, https://milconnect-pki.dmdc.osd.mil/, a new card will not be issued until the data has fed into RAPIDS; no manual entry is authorized. 
  • Name Change: Have two of the following: Marriage License/Certificate, Social Security Card (with new name), or other appropriate court documentation.
  • Expired CAC/ID Cards: MUST bring expired CAC/ID card to DEERS office.
  • Lost/Stolen ID Cards: Consult with your local DEERS personnel.
  • Retirees: Bring your retirement orders to the DEERS office if you have not received a Retiree Card.
  • Always bring two forms of ID when getting issued new card.
  • All supporting documentation is required to be original.
For Military Dependents:
  • Adding a Spouse: DEERS must scan and upload scan of Marriage Certificate, Social Security Card, Driver's License and signed copy of the DD Form 1172. If previously divorced, bring Divorce Decree to remove prior spouse. A new spouse cannot be added until the divorce is verified. The sponsor must sign the DD Form 1172 to add the new spouse. If the sponsor is not available, the spouse may bring a Power of Attorney or the DD Form 1172 fully signed by the sponsor.
  • Replacing ID Card for a Spouse: The spouse must have two forms of identification, which includes at least one photo ID. If a previous card was issued, the old ID must be returned to the DEERS office. The sponsor must be present to sign the DD Form 1172, or the spouse may bring a Power of Attorney or a DD Form 1172 signed by the sponsor.
  • Adding a Child: Bring Birth Certificate (or hospital certificate if child is less than 3 months old). A Social Security Card is also required but a child can be added without it. After 90 days, the child will no longer qualify for any benefits until the Social Security Card is provided. The sponsor must be present to sign the DD Form 1172, or the spouse may bring a Power of Attorney or a DD Form 1172 signed by the sponsor.
  • Adding Stepchildren: Bring Marriage Certificate and Birth Certificate (or hospital certificate if child is less than 3 months old). All other document requirements of adding dependent children still apply.
  • Single Father Adding his Child: To add a child to DEERS as a single father, the father must have the Voluntary Acknowledgement of Paternity (VAP, provided at time of birth at hospital) or court order that establishes paternity and the birth certificate (or hospital certificate if child is less than 3 months old). All other document requirements of adding dependent children still apply.
  • Adding or modifying records for Incapacitated Children: If the child is not yet in DEERS as incapacitated, an original doctor's letter will be required stating the dependent is NOT able to care for themselves or that they need the sponsor's assistance. The doctor's letter must also state if the condition is permanent or temporary. If it is temporary, an anticipated recovery date must be on the letter.
  • Making a new ID Card for a Child: The child must be at least 10 years old with few exceptions. The sponsor must be present to sign the DD Form 1172, or the spouse may bring a Power of Attorney or a DD Form 1172 signed by the sponsor.
  • Dependents in College: Must have an original letter from their college that proves enrollment and reveals an anticipated graduation date. College students can receive dependent ID cards until age 22. The sponsor must be present to sign the DD Form 1172, or the spouse may bring a Power of Attorney or a DD Form 1172 signed by the sponsor. Dependents age 23-26 must contact Tricare and be enrolled in Tricare Young Adult before requesting a new dependent ID.
