Obtaining an ID Card


For Military Personnel:
 
  • For Rank Change: Have Orders because it may not reflect in DEERS and two forms of identification.
  • For New ETS or Enlistment: May contact DEERS to verify the new ETS/enlistment has made it to DEERS or verify in Milconnect https://milconnect-pki.dmdc.osd.mil/ .  A new card will not be issued until the data has fed into RAPIDS; no manual entry is authorized. 
  • For Names Change: Have two of the Following, Marriage license, Social Security Card (with new Name) or other appropriate court Documentation.
  • For Expired ID cards: MUST Have expired ID Card.
  • For Lost Or Stolen ID cards: Consult with your local DEERS personnel.
  • For Retirees: Have your retirement orders if you have not yet received a Retiree Card.
  • Always bring two forms of ID when getting issued new card.  All supporting documentation is required to be the original.

For Military Dependents:
 
  • To Add a Spouse to DEERS: Have marriage certificate, social security card and drivers license. If previously divorced bring divorce decree to remove prior spouse. NEW SPOUSE CANNOT BE ADDED UNTIL THE DIVORCE IS VERIFIED. The sponsor must be present to sign the DD Form 1172 or the spouse may bring power of attorney or a sponsor SIGNED DD Form 1172.
  • To Add a Child to DEERS: Have birth certificate (or hospital certificate if child is less than 3 months old).A social security card is required but a child can be added without it. After 90 days the child will no longer qualify for any benefits until the social security card is provided. The sponsor must be present to sign the DD Form 1172 or the spouse may bring a power of attorney or a sponsor SIGNED DD Form 1172.
  • To Make an ID Card for a Spouse: Spouse must have two forms of identification and one must be a picture form. If a previous card was issued it must be returned. The sponsor must be present to sign the DD Form 1172 or the spouse may bring a power of attorney or a sponsor SIGNED DD Form 1172.
  • To Make an ID Card for a Child: The child must be 10 years old with few exceptions. PLEASE CONSULT DEERS PERSONNEL IF YOU HAVE ANY QUESTIONS. The sponsor must be present to sign the DD Form 1172 or the spouse/guardian may bring power of attorney or a sponsor SIGNED DD Form 1172.
  • College Students: Must have an original letter from their college with an anticipated graduation date and enrollment date on it. College students can receive ID Cards until the age of 21-22. The sponsor must be present to sign the DD Form 1172 or a sponsor SIGNED DD Form 1172 or a legal representative with the sponsors power of attorney must accompany the student.  For dependents 23-26 will need to contact Tricare to be enrolled in Tricare Young Adult and then may come in to get a card.
  • Incapacitated Children: If the child is not yet in DEERS as incapacitated, an original doctors letter will be required stating the dependent is NOT able to care for themselves or that they need the sponsors assistance and if it’s permanent or temporary. If it is temporary an anticipated recovery date will need to also be on the letter.

  • To Add a Newborn to DEERS: Enrollment within 30 days of birth.  Have birth certificate (or hospital certificate if child is less than 3 months old).  A social security card is required but a child can be added without it. After 90 days the child will no longer qualify for any benefits until the social security card is provided.   • To Add a Stepchild to DEERS: Have Marriage Certificate and birth certificate (or hospital certificate if child is less than 3 months old).  A social security number is required but a child can be added without it. After 90 days the child will no longer qualify for any benefits until the social security number is provided. The sponsor must be present to sign the DD Form 1172 or the spouse may bring a power of attorney or a sponsor SIGNED DD Form 1172. CLICK HERE.  
  • To Add a Child to DEERS of a Single Father: Must have the Voluntary Acknowledgement of Paternity (VAP, provided at time of birth at hospital) or Court Order that establishes paternity and birth certificate (or hospital certificate if child is less than 3 months old).  A social security number is required but a child can be added without it.   • For Lost or Stolen ID cards: Must have two current forms of ID as well as an AGO 1000-13 Lost/Stolen Incident Report for CAC ID Cards or a Police Report.
